Simply log in to the Prime Pet Portal
- Upload a copy of both the itemised invoice and the consultation notes from your vet visit.
- If you have not signed up to the Prime Pet Portal before 31 March 2026 you will need to download and complete a Vet Fee Claim Form, and post it to Prime Pet Insurance, Locked Bag 9021, Castle Hill NSW, 1765.
